Grace Eyre offers comprehensive support for autistic young adults and young adults with learning disabilities as they transition into adulthood.
Our services are tailored to meet the unique needs of individuals aged 18-25 in Brighton & Hove and West Sussex.
We can provide support into education or employment, housing and community integration. We can also help with personal development, learning new skills, finding somewhere to live, having fun and making friends.
Transition means moving from childhood to becoming a young adult.
When a young person with additional needs is around 18, they can start moving from children’s to adult social care. However, the planning for a young person’s transition will start at the age of 14.
A social worker can provide more information and advise on what could come next, so speak to them initially. A young person’s school or college may also be able to offer guidance.
When it’s time to transition, Grace Eyre can work with you and your family to make the path as smooth as possible.
We provide support to develop career and life skills.
