Choices
Choices offer supported living and community support to adults with learning disabilities. We work with people who live on their own, with their families, friends or partners.
Our staff work alongside individuals to ensure they receive a safe, professional and personalised support at both home and in the community.
Working together with individuals, families, carers, advocates and social care workers, individuals are supported to create their own support plan tailored to their individual needs and wishes. We also help to facilitate people developing their own Person Centred Plan and Circle of Support.
By providing person centred models of service, we support and encourage people to gain independence and ensure their rights, privacy and dignity are respected.
We think that people using our service should be able to live as equal citizens and support individuals to live to their own chosen lifestyle in a safe and respectful manner.
Choices is registered with the Care Quality Commission. Our staff team receive induction and specialised training, supervision and annual appraisals to ensure they perform to a high standard of safe working practices.
Our staff offer support and assistance in many areas including; specialist and personal care, medication administration, health related activities, budgeting, support at college, work and leisure activities.
We work with people whose funding arrangements include: private, personal budgets, brokers, Independent Living Fund, Supporting People and Social Care funding through their local authorities.
We accept referrals made by individuals, families, carers, advocates, brokers and social care workers.
